Chicago Med broke up one couple and started another this week. Find out what happened in our Chicago Med season 4, episode 16 recap.

This week’s Chicago Med had plenty of moments that One Chicago fans will be talking about, as they drastically changed the way things look around the hospital.

Wednesday’s installment “Old Flames, New Sparks” featured Dr. Daniel Charles (Oliver Platt) in a hard place while his ex-wife was treated for cancer, which left her fate in the hands of Dr. Connor Rhodes (Colin Donnell). Connor, being Connor, took drastic measures to save the day.

Meanwhile, there were major moves for One Chicago shippers: one couple called it quits while the long-rumored love triangle finally came to pass.

Charles and Connor

The main story of “Old Flames, New Sparks” was introduced as a subplot last week: that Charles’ ex, Caroline (guest star Paula Newsome, NCIS) had arrived at the hospital after her health took a turn for the worse.

Caroline went to be evaluated to see if she could participate in a clinical trial. But the physical exam revealed a heart problem—which not only kept her out of the trial but meant they needed to call a cardiologist. Luckily, they knew one; they immediately paged Connor, who diagnosed Caroline with heart failure, likely due to her chemotherapy.

With just two weeks to solve the problem so Caroline could get into the clinical trial, Chicago Med sent Connor down a familiar path: considering a risky surgery. Though he said no, Dr. Ava Bekker (Norma Kuhling) overheard and decided to take over the case—even calling Connor “gunshy” due to his past relationship with Robin (Mekia Cox).

Though Sharon Goodwin (S. Epatha Merkerson) chastized Ava for her unprofessional behavior, she didn’t stop her from scheduling the surgery. An infuriated Connor decided that if someone had to potentially kill Caroline, it was going to be him. And an awkward Charles stumbled over trying to tell his ex-wife that he still loved her.

Connor and Ava continued to argue in the prep room and in the OR. At this point, we should ask if there’s a Chicago Med episode they haven’t fought or snarked in. But when Caroline had cardiac arrest, it was Connor who got her back. She pulled through the surgery, and while Robin went off to be with her mother, Connor told Ava “don’t you ever put me in that position again.” Preach.

On a happier note, Caroline not only got into the clinical trial, but Goodwin also hinted that Charles’ feelings for her might be reciprocated. Hey, someone on this show can be happy, right? Right?!
