After an intense two weeks of episodes wondering about Dr. Charles' fate, Chicago Med season 11, episode 18, "Things Left Unsaid," looks to turn the page as Charles officially makes his way back to work.

That's right, Charles will officially be returning to work in this week's new episode, hinting that perhaps a small time jump will have occurred between episodes 17 and 18. Details about Charles' first day back in the ED following his stroke are being kept under wraps with no mention of Charles' storyline for the episode; however, we do know that Theo will also be back this week, and we're worried the show is about to begin laying the groundwork for the conflict ahead for the pair.

It was recently teased by Chicago Med showrunner Allen MacDonald that Theo is about to pose a major threat to Charles' job. He kept the tease general, leading us to wonder if perhaps Theo might try to come for Charles' job as chief of psychiatry and behavioral health at Gaffney.

After watching Theo and Charles becoming a team over the course of the season, we're not looking forward to Theo suddenly becoming a threat to Charles' job. Our hope that this threat is not one Theo initiates, but that perhaps the board might look to promote Theo after Charles' health crisis in looking to convince Charles to take a step back due to concerns with his health.

Speaking of concerns, episode 18 will once again see concerns being raised about Lenox's decision-making.

One of the central storylines set to be featured in the episode is the fallout of a horrific car accident that puts a family's fates in the hands of the Gaffney staff. Among the doctors working the case will be Ripley and Lenox, and it seems that tensions are about to rise between the couple when Ripley begins to question Lenox and her reckless behavior.

As the promo for the episode teased, the root of the concern seems to stem from Lenox proposing a risky surgery on a young boy injured in the accident. We see Lenox presenting the course of treatment to the boy's father, and it becomes clear that Ripley is not on board with her plan going as far as to tell her, "I'm not going to let you kill that kid," as he storms out of the scrub room.

This storyline will also bring back a familiar face with Jodi Kingsley reprising her role as DCFS Officer Madeline Gastern, marking her first appearance in season 11. Given the car accident involved two children - the promo for the episode reveals that the man's daughter passed away as a result of the accident - it's not surprising that Madeline is brought in as part of the storyline, but we're certainly eager to see why DCFS ends up being called to the hospital for this case.

It also appears we’ll be getting some sort of storyline update on the Frost and Naomi front. There is little context as to what might be coming our way for the pair this week, but in one still from the new episode we see the pair engaging in what appears to be an intense conversation.

Could it be that Frost finally comes clean to Naomi about his feelings, and Naomi’s body language in the photo is hinting that she’s not so happy with him dropping that reveal on her now that she’s in a healthy relationship? That would certainly be an understandable reaction for her to have to Frost professing his feelings now that she’s finally moved on.

We'll find out when the episode airs on April 22 at 8/7c on NBC!
