It's time for another new episode of Chicago Med season 11 on NBC! So far, the new season has been on fire with some of the show's best surprises in recent years. For the 200th episode, the medical drama welcomed back Nick Gehlfuss as Will Halstead and shocked everyone by keeping the secret of Torrey DeVitto's return as Natalie Manning. The incredible season continues tonight!
Last week's episode dealt heavily with Halstead and Manning's send-off, which included the Gaffney team working together to help save Owen from his near-fatal gunshot. Natalie ended up risking her pregnancy in order to donate one of her kidneys to her son. Natalie, Owen, and Baby Halstead all pulled through and the little family was happily headed back home to Seattle.
Chicago Med season 11 episode 4 returns to business as usual with the current Gaffney team taking on new patients and tackling more hurdles in their personal lives. What time is Chicago Med season 11 episode 4 on NBC?
Chicago Med season 11 episode 4 airs on Wednesday October 22, 2025 at 8/7c on NBC. The episode remains in the show's usual timeslot on One Chicago Wednesdays and should begin as normal without delays, kicking off another night of new episodes. Chicago Fire season 14 episode 4 airs at 9/8c and Chicago PD season 13 episode 4 closes out the night at 10/9c.
For those who aren't able to watch the episode live on NBC or with a live streaming platform like Fubo, you can stream the new episode the next day on Peacock. If you have the Peacock Premium Plus plan, you can even watch the episode live at the same time it's airing on NBC, but with any of the streamer's other subscription plans, you'll have to catch up on the new episodes on Thursdays.

What to expect in Chicago Med season 11 episode 4
Seeing as season 11 episode 3 pretty much focused on Halstead and Manning and wrapped up a two-part story about Owen's shooting, we're going into episode 4 with something of a fresh slate, or at least returning to the characters we know and love. Just like with every episode, Gaffney's emergency department doctors are taking on new patients and challenges.
Before the episode's airing, NBC shared the official synopsis for Chicago Med season 11 episode 4: "Charles races to convince a patient to accept treatment before it’s too late. Frost bonds with a young girl battling immunodeficiency. Lenox makes a chilling discovery."
According to the synopsis, Charles (Oliver Platt) and Frost (Darren Barnet) will both be taking on what could be the biggest patients of the episode. Charles isn't a stranger to helping convince patients to accept what they're going through or take on treatment that might be scary, and that's exactly what he's doing this week. Likewise, Frost will be treating a young patient and putting his heart into bonding with her.

Manish Dayal guest stars in Chicago Med season 11
In addition to introducing new patients and continuing storylines from the beginning of the season. Chicago Med season 11 episode 4 will also welcome a new guest star. The Resident alum Manish Dayal will appear in the episode as Dr. Theo Rabari. While some know him as Dr. Devon Pravesh (and real ones know him as Raj from 90210), we'll see Dayal at Gaffney as a new doctor!
Meanwhile, there are quite a few character moments we can look forward to in this week's new episode. Lenox (Sarah Ramos) has been battling her GSS diagnosis but not completely facing it head-on. She's been avoidant, but each episode has brought her closer to the realization that she could and should do something. There's a "chilling discovery" in store for Lenox, but is it about her health?
In the wake of Asher's (Jessy Schram) shocking pregnancy news, Archer (Steven Weber) and Ripley (Luke Mitchell) were forced to work together in spite of their mixed feelings for each other. Are there any new developments about Asher and Archer? What about Ripley's relationship with Sadie? The episode's preview trailer only teases Archer's patient who severed his own hand and doesn't want it reattached. What?!
