What’s going to happen in the Chicago Med season finale? Check out our predictions for Wednesday’s last episode of Chicago Med season 4.
The Chicago Med season finale is less than a week away, and One Chicago viewers are already waiting on the edge of their seats to see how everything concludes after what’s been a season of dramatic ups and downs.
Wednesday’s installment will have a lot to resolve, including another love triangle (which may or may not be a square) for Manstead, another attempt at a wedding, and more suspicious behavior around the hospital.
The finale will also be the last episode for Colin Donnell and Norma Kuhling, as the actors were dropped from the show by producers and won’t be returning for season 5. So how is it all going to shake out?

1) Natalie will find out something about Phillip
Dr. Natalie Manning (Torrey DeVitto) is head over heels for her boyfriend Phillip Davis (recurring guest star Ian Harding), but expect that something’s going to come out in the finale that may change that.
Chicago Med producers hinted a few weeks ago that something “disturbing” would be revealed about him, but it hasn’t actually happened yet. Dr. Will Halstead (Nick Gehlfuss) caught Phillip in a lie about owning a ranch, but that’s not a revelation that would qualify as “disturbing.”
Plus, these last few episodes have shown a real lack of clarity when it comes to Manstead. Not only is the Phillip storyline unclear, so is whatever the show was planning with Will and FBI agent Ingrid Lee (Anna Enger). Was that supposed to be another part of this love triangle? Even if Will and Natalie stay separated, there’s no clear idea of where either one are actually going.
Having a bombshell come out about Phillip would fulfill the producers’ tease and also give this whole storyline a direction to head in for next season.

2) Ethan and April get stuck with Emily’s baby, again
Last week’s episode “Forever Hold Your Peace” saw Emily Choi (recurring guest star Arden Cho) almost abandon her son Vincent. We’re not convinced she won’t do it again, or that somehow the baby won’t end up taken away from her.
Chicago Med season 4 spent far too much time on Emily and her ridiculous boyfriend Bernie, and in the process made a mess of the storyline between Dr. Ethan Choi (Brian Tee) and April Sexton (Yaya DaCosta). Half of what Choi and/or April have had to deal with this season hasn’t even been about them, and then Choi has been made to look like the bad guy for insisting that Emily and/or Bernie get their stuff together.
Emily’s storyline is now in its second season, and it’s hard to see it persisting into season 5. There just isn’t enough left to tell. And if Bernie hasn’t improved by now, it feels unlikely he ever will. It’d make perfect sense for Chexton to put their foot down and say the baby is better off with them, or no surprise at all if Emily decides to head off to St. Louis for real this time.
After all, sticking April and Choi with baby Vincent would be a big dramatic speed bump for their renewed romance to navigate next season, right?
3) Connor will leave Chicago
Fans know Dr. Connor Rhodes will leave Chicago Med in the season finale, since Colin Donnell has been cut from the series. But we’re going to take it one step farther and say he’s going to leave the city, too.
When Connor was first introduced, he was coming home to Chicago after having spent time away, and that was part of the initial arc for his character—returning to his home city and having to deal with all of the baggage he left to get away from.
But this season has piled even more baggage onto him. Connor’s difficult father Cornelius (guest star D.W. Moffett) was killed off last week, and Chicago Med now has him questioning everything he knows about his family history. Plus, his ex-girlfriend Dr. Ava Bekker has been made to look like a sociopath, possibly a murderer.
It would be an appropriate bookend to his story if Connor decided he needed to move on from the Windy City—maybe leaving with his other ex-girlfriend Dr. Robin Charles (Mekia Cox), since she got a bit of a raw deal being passed over in favor of Ava in season 3? But if he said, “I can’t be here anymore and I need a fresh start,” nobody would blame him.
