As the penultimate episode of Chicago Med came to an end, it was clear that the season 11 finale would feature some major developments and be a can't-miss episode for the fans.
In the closing moments of the episode, Gaffney went into lockdown as a Code Silver was called after a prisoner escaped and made off with a gun. This set up what we knew would be a heart-pounding episode exploring the fallout of the code silver while juggling the other storylines set up by the penultimate episode, including the revelation that Theo might be a sociopath; Dean ready to confess his love for Hannah only for her to end up going in labor while in lockdown helping a patient who had also gone into labor; Lenox telling Ripley she loves him; and Frost learning that his father was in hospice.
The season 11 finale begins with Hannah working to help Olivia to deliver her baby, but it becomes apparent she's not going to be able to do this alone as she tags in Jacob to take over as her own contractions intensify.
We jump to Lenox and Ripley in the fallout of her profession of love. While Lenox was hoping for an "I love you too," Ripley makes it clear he has a lot to think about and her "I love you" hasn't erased all she's put him through. As he points out, she wants to do whatever she wants, whenever she wants, and never seems to think of others around her and what they want. He calls her infuriating and makes it clear that he can't go along with her in pretending she's not sick. CPD comes through to clear the area and orders them to remain on the floor until the code silver is lifted, and Ripley can’t leave fast enough.
Elsewhere, Charles tells Goodwin about his suspicions that Theo is a sociopath as she questions whether he can prove it. He mentioned an fMRI could prove it as she reminds him the board will be meeting soon to determine whether Theo will replace Charles full time. Goodwin suggests Charles use this card against Theo as she reminds him that Theo had no issue exploiting Charles’ health scare, but he refuses to do so even if it means saving his job. Goodwin makes it clear she can’t afford to lose him and they don't have much time left to act
In the staff locker room. Dean bumps into Naomi and asks if they've cleared the labor and delivery floor, as he can’t get in touch with her. She assures him they're working to clear the hospital floor by floor as she leaves. While grabbing his medical coat from his locker, Dean spots Wyatt, the escaped prisoner, in the reflection of his locker mirror and tries to leave. Wyatt comes up to Dean and tells him he’s going to help him finish something, and he might let him live to meet his baby if he plays along.
After taking a moment, Hannah steps back in to deliver Olivia's baby successfully. CPD reaches the floor and lifts the lockdown. Lovell arrives with Jacob’s partner to introduce them to their son as Lovell picks up on Hannah's contractions coming close as she prepares to take her to deliver.Â
Goodwin greets Miranda and asks to postpone the board meeting, as she wants to have Theo complete the same tests that Charles had to complete for fairness to put them on equal footing, but Miranda makes it clear there is no delay. Goodwin suggests that Miranda put the idea in Theo’s head to make the position fully time as she excuses herself.Â
Naomi goes to see Jason Walker, the prison guard involved in the fight that led to the riot, and he begins to strangle her. She punches him as Lenox rushes in, as Jason admits he thought she was trying to kill him. Lenox checks on Naomi as she asks if she’s called Frost, and she mentions that he’s away visiting his father. Naomi tells Charles about how Jason thought she was Wyatt and was going to kill him as she reveals how many of the inmates suggested the attack against Maxwell was instigated by Jason.
Doris informs Charles there is a man named Scott who has called from inpatient treatment about seeing Charles, and he offers to touch base with the man when he has a minute as he goes to see Jason. Charles points out how Jason has an elevated baseline of fear, and Jason admits he thought Maxwell had a shiv, but he realizes there wasn’t one and that if Maxwell dies, he can’t live with himself. Thankfully, Maxwell’s going to make a full recovery, but Charles tells him that there is help out there if he sees it.Â
Hannah is settled into a room, and it’s clear that she is not used to being the patient. She presses Lovell about her stats and finally comes clean about her fears and tells Lovell about her mom dying in labor. Lovell assures her she’s not going to let anything bad happen to her or the baby and asks her to trust her. Goodwin arrives to check in but Dean hasn’t been up yet as she tells her to focus on the baby before calling in security.Â
CPD tracks down the location of Dean’s last badge scan, but inside the locker room the only thing they find is Dean's badge and wallet, which Goodwin notes he would never leave.Â
Charles goes to see Scott, and it's clear that Scott knows Charles, but Charles does not seem to recognize Scott. That's when Scott admits that they never met before. He reveals they had a pretty intense conversation, and we learn that Scott is Gio, the young man who called the helpline earlier this season whom Charles suspected had killed himself. It turns out that Scott came to the hospital the day Charles had his surgery, and Theo has been helping him a lot in their sessions as an inpatient. Charles is thrilled by the progress as Gio notes that he was coming to the hospital to thank him for saving his life, and Charles reminds him that he did that.
Elsewhere, Lenox is treating a man who was shot in the head, and Ripley comes in to assist. She's called away when Kip arrives and finds him raiding the lounge for food. He admits that he saw the news about the prison riot and came to check on her, as she's the only family he has left. He can sense something is up, and he asks her to spill it as she admits she lied to him last year about her GSS test. Lenox finally comes clean and informs him she tested positive and hid the truth, as she didn’t want him to worry. After an angry outburst, Kip embraces her as he cries, the reality of her statement sinking in.Â
We finally spot Dean as he walks into Isaac’s room, where he finds Ripley, and he’s not alone; Wyatt is with him. Wyatt orders Ripley to wake Isaac up, but Ripley informs him there is no way to wake him up, as Isaac had some complications and is brain dead. Wyatt leans in and tells Isaac to breathe so they can get out and live the life they planned when they escaped. It seems Wyatt was not coming to kill him but to free him so they could run away together, as the pair had been romantically involved.
After police arrest Wyatt, Dean makes it to the delivery room as she jokes about him better being in mortal peril as she begins to give birth, not realizing the danger he was in. Thankfully, the delivery goes smoothly and their little girl arrives without any complications. Later, Dean and Hannah are in the room with their daughter, Mabel, and Dean finally tells her he loves her and has for a long time, even before Mabel. It wasn't just one moment; it was thousands of moments that revealed to him how much he loves her. She admits she couldn’t process it as she thanks him for telling her now and pulls him in for a kiss.Â

Charles is keeping his job but Goodwin's job is on the line
Charles goes to see Theo, and he assumes Charles is going to tell the board about his sociopathy to save his job. Charles assures him he won’t be leveraging this against Theo, as he doesn’t want to destroy his career and revealing the information to the board would go against everything he’s done. While he won't be using the findings against Theo, Charles notes that he built this department from the ground up and won’t be backing down from anything.Â
Goodwin and Charles head to the boardroom, but Miranda informs them that Theo has removed himself from the position for personal reasons as she notes that she would like Charles to resume his position. She then informs Goodwin that she is leaving Gaffney. Goodwin is shocked as Miranda notes she's giving her the opportunity to leave after she leaked Theo’s medical records to her. Goodwin makes it clear she doesn’t frighten easily as Miranda gives her an ultimatum to leave within the hour. After she leaves, Goodwin assures Charles the less she says, the better.Â
Charles goes to see Theo and stresses he had nothing to do with leaking his file. Theo assures him that he was not the one to flag Charles' fMRI scans and that he was simply asked to take a look at the results. Charles asks why Theo never told him about Scott, and Theo admits he wanted to prove he could do the talk therapy aspect of the job. Theo assures Charles he’s going to be okay as he heads out, possibly for good, as the moment felt very final.
As the episode ends, Ripley goes to see Lenox after hearing about the blowup she and Kip had. She admits she told Kip the truth as she remarks that it’s easier to be the sick person, as her brother taught her. She admits that she’s too okay, and speaking of honesty, she would like to offer Ripley a guilt-free breakup. To her surprise, he rejects the offer. She doesn’t want to ask him to run up the clock and take care of her, and he admits that he would rather have another day with her than a lifetime with someone else. She can’t pretend he’s not in love with her as they kiss.Â

Frost's dad dies in the Chicago Med finale
Elsewhere in the finale, Frost goes to see James, and it’s clear that he doesn’t have long. Frost can see his mom is not okay as he encourages her to get some rest and allow him to stay for the day. James begins to struggle to breathe as he calls for supplies. He heads to the crash cart to search for a needle and runs into the room and injects it into his side, draining fluid that is gathered to help with his breathing.Â
Frost hears James calling out for Celeste and tells him he sent her home to get rest. James is surprised that Frost came, as he asks him to forgive him, and Frost assures his dad that he has forgiven him. He curls up in bed beside his dad as he tells him it’s okay. Frost assures his dad that it’s okay to let go and he doesn’t have to fight anymore as his dad takes his final breath.
As the episode draws to a close, Frost admits that he didn't think he loved him, but the truth is he never stopped loving him as he and his mom embrace.Â
