Now that Chicago Med has finished another season, it’s time for our annual post-season ratings review, where we look at something else that’s important to the show—its ratings numbers.
One Chicago fans know that as much as we love the show, TV is also a business, and that comes down to ratings. We’ve been providing you with weekly ratings reports throughout the season, but now it’s time to look at the big picture and see what that tells us about season 4.
Overall numbers
The Chicago Med season 4 premiere had 7.78 million viewers, which made it the highest-rated start to a season since season 1 (which had 8.64 million in November 2015).

The show experienced a lift of 1.59 million viewers over the previous season’s premiere (that was 6.19 million in November 2018).
The season finale in May was an even bigger improvement—it recorded 7.55 million people who tuned in, which was almost a full two million more than watched the season 3 finale (5.62 million)!
Overall, Chicago Med had its most successful season ever. It was the 15th most-watched show during the 2018-2019 TV season, based on Live +7 viewing.
If you’re not familiar with that term, it means live viewers plus everyone who watches on demand or DVR over the following week. Previously, the show hadn’t ranked any higher than 27th in that category—so it picked up an impressive dozen places on that countdown!

Lowest-rated episode
The least-watched Chicago Med episode this season was “With A Brave Heart” which had just 7.55 million viewers. That’s a shocker, because that’s the season finale—one of the two episodes that are usually the most highly rated in a TV season, since they’re the big episodes where a lot of things happen!
Clearly, audiences weren’t as invested in the resolution to the Connor and Ava storyline, or Will having more issues with the Burke family. Perhaps that’s a sign that this season stretched those storylines out a little too long, and viewers were getting tired of them?
Highest-rated episode
The most-watched season 4 episode was “The Things We Do” which brought in 9.41 million live viewers. That’s episode 12, which was primarily about a helicopter crash with Dr. Natalie Manning (Torrey DeVitto) on board.
The promos for the episode did an excellent job of suggesting Natalie could be killed in the crash, and pretty much exclusively were about that, so viewers were clearly wanting to tune in and see whether or not she survived the crash. Which she did, but now who knows if she’ll make it out of the season finale?
