Where we left off
Dr. Will Halstead (Nick Gehlfuss) proposed to Dr. Natalie Manning (Torrey DeVitto), but the season finale ended before she gave an answer.
And it could go either way, since just minutes before Will proposed, Natalie realized who it was Will had been drunkenly kissing earlier in the season. Don’t miss our pros and cons of the Manstead proposal.
Dr. Ethan Choi (Brian Tee) found and saved his sister Emily (recurring guest star Arden Cho), but April Sexton (Yaya DaCosta) told Choi he’d have to choose between her and his sister.
Dr. Connor Rhodes (Colin Donnell) got a job offer at another hospital, Sharon Goodwin (S. Epatha Merkerson) got told she’d have to work with a new hospital executive, and Reese’s dad Robert Haywood had a heart attack.

Who’s out
- No one left the show between seasons (making Med the only show that didn’t write out a character in its finale), but a main character will leave during the season premiere.
Who’s in
- After guest starring in the third season finale, Heather Headley (The Bodyguard) will recur during the season as Gwen Garrett, the hospital’s new Chief Operating Officer.
- Two new medical students will be introduced in the season premiere and recur throughout the season.
What we know so far
- Like the other two One Chicago shows, the Chicago Med premiere picks up only a short time after last season’s finale.
- The season opener will include the departure of a main character, though we can’t tell you who (spoilers!)
- The first episode will answer the cliffhangers about Will’s proposal to Natalie and whether or not Robert has survived his heart attack.
- The second episode will be a crossover episode with the rest of the One Chicago franchise.
Don’t miss the Chicago Med season 4 premiere next Wednesday, Sept. 26 at 8 p.m. on NBC. For more news throughout the season, follow the show category at One Chicago Center here.
