Just like that, another season of Chicago Med has come to an end, and fans can (for the most part) breathe a huge sigh of relief. While the season 11 finale did feature an emotional death, the main characters made it through the season unharmed and ready to continue saving lives in season 12. But even though the finale didn't feature an exit, that doesn't mean there won't be one.
In Chicago Med season 11 episode 21, Gaffney fell under a Code Silver when one of the prisoners they were treating escaped. While Dean was held at gunpoint by said prisoner, Hannah went into labor while delivering a patient's baby. Throughout the season, showrunner Allen MacDonald teased something could go wrong with Hannah's pregnancy, but thankfully she and baby Mabel are healthy.
Dean also wasn't harmed by the armed prisoner, though his near death experience before welcoming his daughter definitely rattled him. However, Frost's father passed away in a heartbreaking scene that brought their story full circle. Going into Chicago Med season 12, it seems as though the full main cast will be back, though MacDonald recently hinted that a shakeup could be on the way.

Speaking to TV Insider following the Chicago Med season 11 finale, MacDonald was asked whether he could confirm if everyone in the main cast would be back in season 12. Unfortunately, the showrunner didn't quell our concerns, but he instead said seven words that only has us worrying even more about a potential cast exit: "I am not at liberty to say."
Naturally, you would assume that if the entire season 11 main cast — S. Epatha Merkerson, Oliver Platt, Steven Weber, Jessy Schram, Luke Mitchell, Sarah Ramos, and Darren Barnet — were back on board for season 12, he would have simply said yes. After all, what's the downside of confirming the cast is back when the season didn't end with anyone's lives in mortal danger?
But as we know with the teasing about Hannah's pregnancy, MacDonald likes to keep fans on their toes and never let us feel too comfortable. In that regard, he could simply just be stoking the fires of fan fears or he really is keeping mum on a potential cast shakeup. As of the season 11 finale, there has been no official word announced of a main cast member stepping down in any capacity.
In fact, ahead of the season 11 finale airing, reports surfaced that Merkerson and Platt had secured new deals to return as Goodwin and Charles in season 12. Despite the cliffhanger that Goodwin's job could be at risk, we know that Merkerson will be back next season. Additionally, Schram spoke in the TV Insider interview with MacDonald and referenced what Hannah will be up to in season 12.
Therefore, we can pretty much bank on three main cast members returning. Still, given the ending of season 11, there's no real reason we would expect Weber, Mitchell, Ramos, or Barnet to depart the series for any reason. There's a possibility that Ramos could be on maternity leave after announcing her pregnancy in May 2026, but that would be a temporary leave of absence.
Lenox and Ripley appear to be in a great place in their relationship, and even though Lenox finally broke the news about her positive test result to her brother Kip, there's no reason to believe that Lenox's diagnosis will catch up with her that quickly. Frost, too, doesn't have something pulling at him outside of Gaffney, apart from the possible sitcom reboot, but he and Naomi are solid together.
Again, MacDonald's tease about the series regulars could mean nothing, and we could most likely be seeing everyone back on our screens as normal this fall. But there's always the possibility that a cast shift could be on the horizon. There's an even bigger, and almost guaranteed, chance that Chicago Fire could be saying goodbye to a main character next season. Welcome to One Chicago!
